“No evidence of an equipment failure”
The Royal Canadian Air Force say that their investigation into the death of 31 year old Master Cpl. Alfred Barr in a training accident was likely a result of human error and not equipment failure.
According to a preliminary report the RCAF say that there was no evidence of an equipment failure and also the weather conditions on the day of the parachute jump was clear over the drop zone.

The report says that as Barr jumped out of the plane his left leg moved upwards and his body rolled to the right as the static line started to open his parachute causing his chute to not open properly resulting in him spiralling as he descended.
Barr was unable to stop the spiral by untwisting his lines. He died instantly as he hit the ground.
Barr was based, and lived. in Winnipeg .
